The following forms must be completed by all financial aid applicants:

• Application for Financial Assistance
• Satisfactory Academic Progress Requirements Form
• Print Financial Aid History Instructions (NSLDS)

NOTE:  Students are highly encouraged to use the IRS Data Retrieval Tool on the FAFSA.  Using the tool will save time and paperwork.  If you would like to use the tool, login to your FAFSA.  Click "Make FAFSA Corrections", and follow the instructions in the Financial Information Section to download your tax information directly from the IRS. 

Students may also be required to complete:

• Appeal Form
• Academic Plan
• Tax Return Transcript  and Proof of Nonfiling Instructions
• Exit Counseling Instructions
• Statement of Student Eligibility
• 2012-2013 Dependent Verification Worksheet
• 2012-2013 Independent Verification Worksheet
